Terms:

  1. Photocopy of Student Identity Card (KTM);
  2. Photocopy of Study Result Card (KHS) Odd semester 2018/2019 academic year
  3. Photocopy of Study Plan Card (KRS) even semester 2018/2019 academic year
  4. Photocopy of the front page of the BNI Bank savings book or print out the bank statement for those who do not have a BNI KTM Plus;
  5. Declaration letter from the head of the village where the parents of the prospective scholarship recipients are domiciled stating that the person concerned is truly unable to pay for the education of their child whose name is listed on the letter
  6. Photocopy of family card
  7. Statement Letter
  8. For those who extend, only complete point No. 1,2,3,4 and 7
