GENERAL REQUIREMENTS
1. USK student at D3/S1 level 2017-2020 who comes from underprivileged families, GPA min. 3.00
2. Have academic/non-academic achievements (last 2 years)
3. Don’t date and don’t smoke
4. Have a recommendation from WR III/WD III/USK Lecturer (choose one)
5. You must subscribe to the USK Jamik Mosque Charity House’s YouTube and Instagram for updated information

2. Statement Letter
3. Scan the latest transcript
4. Scan KTM
5. Scan KTP
6. Scan of Academic/Non-Academic Achievement Certificate (last 2 years)
7. Color passport photo size 3×4
PROPOSAL MECHANISM

a. Scan files at points 1-6 in one PDF file (max 1 MB)
b. Color passport photo size 3×4 in JPG format (max 1 MB)
